The 6 extra buttons are hard-coded to keyboard, either numpad 1-6 or numrow 1-6. There is a switch under mouse to select wich one you want to use.

You just need to remap keys ingame to use them all. Simple, effcient, works everywhere with everything.
Also works great with Guild Wards 2 since you mainly use skills 1 to 6.
