Are you sure that you've changed the SD output resolution? Unless you've changed it for the game through the gear icon (gear->settings->resolution) you'll only get automatically scaled output.
If overriding that manually in Quake4Config.cfg, to "unlock" that, you also have to do
seta r_mode "-1"
Then you can set any arbitrary values for custom height and width (i.e. 1080 and 1920)
I'm mentioning this because I forget this almost every time I install one of these old quake engine games again and edit those values.

Thanks Klaas, not sure how I missed that! I selected 1920x1080 (gear→Properties→General→Game Resolution) When the game was launched under in-game settings the option for 16:9 1920x1080 was available which previously wasn't there, so all good.
